Session Overview and Learning Objectives
Advances in paediatric and specialist care mean that more people than ever are living well into adulthood with childhood-onset chronic conditions. As a result, acute care teams increasingly encounter patients whose physiology, treatments, communication needs and healthcare journeys differ from the “typical” adult medical patient.
For many, an admission to the Acute Medical Unit is one of their first encounters with adult acute healthcare, making it essential that acute clinicians understand the challenges of transition and adapt their approach to meet individual patient needs.
The challenge is not becoming an expert in every rare condition. It is recognising when our usual approach needs to adapt.
Using three contrasting but complementary examples, this webinar explores how altered physiology, advanced therapies and medical technology, and communication needs influence the care of adults living with lifelong conditions in the acute setting.
By the end of this webinar, participants will be able to:
– Recognise the increasing number of adults living with childhood-onset chronic conditions presenting to acute medical services.
– Describe how altered physiology, advanced therapies and medical technologies, and communication needs may require adaptation of the usual acute medical approach.
– Identify practical approaches to the assessment and management of adults with congenital heart disease, diabetes technology and learning disabilities in the acute setting.
– Recognise when specialist advice and multidisciplinary working are required to optimise patient care.
– Apply principles of personalised, patient-centred care to improve outcomes for adults living with lifelong conditions.
